Appointment Cancellation Policy
 
Scheduled office appointments which are canceled with less than 24 hours notification maybe subject to a $25 fee this fee will also apply to missed appointments. The cancellation and no show fees are the sole responsibility of the patient and must be paid in full before the appointment.
